Boulder's ID requirements

To get care, you need to show one of these government IDs with a photo:

  • State ID
  • Driver's license
  • Passport
  • Department of Corrections ID
  • Military ID
  • Permanent resident card

If your photo ID is expired, you need to show one more non-photo ID from the list below.

If you don't have a photo ID, you need to show two of these non-photo IDs that match:

  • Social Security card
  • Birth certificate
  • Insurance card
  • Utility bill
  • Bank statement
  • Paycheck or government check
  • W-2 form
